My Diabetes Wellness PortalWelcome to My Diabetes Wellness Portal, an innovative health information and management website for patients of Dr. Merrill Edmonds at St. Joseph's Health Care, London. The My Diabetes Wellness Portal program aims to enhance diabetes patients' connection to the resources and people that can assist with their diabetes management. Participants will have access to a secure portal that provides a range of tools to facilitate learning, self-management, and peer and healthcare provider support. My Diabetes Wellness Portal launched in September 2009 as part of an evaluation of the benefits of Internet-based education and health management for patients of Dr. Edmonds' clinic. The evaluation of My Diabetes Wellness Portal is being led by Dr. Femida Gwadry-Sridhar of the Lawson Health Research Institute and I-THINK Research, and Dr. Edmonds of St. Joseph's Health Care, London. The project is funded by a grant from the Health Technology Exchange (htx.ca), an agency of the Ontario Ministry of Research and Innovation. |
